|  | | | How are transition services provided?
Transition PLUS staff members work in partnership with a variety of
community resources and agencies including community and technical
colleges, Division of Rehabilitation Services, and Ramsey County Human
Services.
Transition PLUS Services employs nine transition
instructors, five educational assistants, two sign language
interpreters/educational assistants, two teacher assistants, and a
secretary.
Staff members help students access vocational
training, postsecondary education, recreation and leisure programs,
public transportation, social services, and housing facilities that
promote independent living.
Ongoing support is provided through job coaching, tutoring, monitoring, and follow-up to ensure student success.
